Being able to identify which patients end their care prior to the recommended plan of care on file is imperative. This would help us identify which therapists are most effective at educating their patients and helping the patient reach a true discharge appointment, as opposed to an administrative discharge (quick discharge, etc) as the patient has not returned to complete their plan of care as listed in the note.
