Remove the extra questions from the Quick DASH form

The Quick DASH is a standardized test. If you add extra questions, as has been done in this software program, it is no longer a valid test. Please remove the extra questions, including the one about pain at the beginning of the test.

It is not appropriate to amend a standardized test with questions about patient satisfaction. At that point it invalidates the entire test.
