All data entered into WebPT should be exportable and reportable

Please see ticket 00458998

There is a field for State Medicaid # and I would like to export that number along with the demographics of the physician for multiple physicians, not just one by one. The response to my ticket was: there is no option to export the State Medicaid# to Excel.

I should be able to query every data element I input for business operations.
