I would like to see a one page, concise document that we could send to physicians. Each sub category in our initial eval/ progress report could have a check box next to it to indicate that this information should be included in the physician summary document. At the end of the documentation we could indicate whether we wanted to create a physician summary document that would also have a signature line for the physician as well.
