Ability to select a duration of appointment based on appointment type

Evaluations are customized to 60 min and standard appts can be customized to 15 or 30 depending on the market. Currently, WebPT allows 1 default duration to be selected(typically follow-ups based on frequency of use) and all other appointment types must be altered on an as-needed basis.
