Problem list and Goals: It is challenging that goals are linked to a problem. I feel like many goals we set address more than one problem. And I also feel it makes it difficult to read. Also often there are mor problems than actual goals we set. I think it worked better when it was problem list and then goals.

Hello! With SOAP 2.0 you have the flexibility to document either way-- you could list all of your goals under one problem (while still having your separate problems listed) if that is easier for your clinic, or you could separate the goals out with individual problems. Hope this helps!
