Move "sign note" button away from the ellipses for other options

It is really easy to accidentally hit the "sign" button instead of the ellipses when the intention is to forward the note to a supervising or co-signing therapist and then an addendum has to be made.

    Hi and thanks for your feedback. This is something we'll consider as a future enhancement. One area that we're looking into is how intuitive it is to have the choices in the elipses and this would play into that. Thanks again.
