It often happens in our practice that we finalize a psychotherapy session but need to add more clinical information later. Maybe we remember some other aspect of what happened, or maybe we want to document a phone call. Since we usually don't bill for phone calls, we don't want to create a session for them. But we absolutely need to document the call. When this happens we have been going to the last session and using Addendums to document the information. But I recently learned that, when searching for all the documentation for a client under Documentation History, you can't see Addendums, nor can you even see which sessions have Addendums on them. I also learned that even when you open up the documentation for a session that does have an addendum, you won't see the Addendum. The only way to know it is there is to download the session and then open it, which you wouldn't know to do in the first place since you don't know which sessions have Addendums. What is the point of documentation if you can't find it or see it? Can you make it so that Addendums are apparent when viewing the Documentation History, and so the clinical info can be accessed without a download? Some of our Addendums are our most critical pieces of documentation, such as calls regarding suicidal clients. If a phone call is made but not apparent in the record that is a huge potential liability for use and it's wrong to not be able to see the entire record online. Perhaps if there is no way to make this happen there is a way to add a second piece of documentation to a session and finalize a second time? Nothing would be changed in the first set of documentation; it would be an add on to the first. Could you let me know via phone call if you need further information or decide to proceed on this?
