Ability to enter: 1) Reqst date (for reporting) 2) note in Authorization tab

We have multiple disciplines who request referral extensions - It would be much more efficient if: 1) the date of the request could be a field so that a report could be run to allow for efficient follow up with carrier / parents /PCP etc 2) if note could be entered by to allow for when follow up done or why there is something different about the auth request that needs a note entered
