remove columns and/or rows in the expected rates schedule

It would be more user friendly of the expected rates schedule used the entire page:

1 .scrolling across of down you lose your column/row headers and difficult to keep track of where you are at

2. delete columns or rows as needed

3. a effective date range as reimbursements change
