Payment and Adjustment columns need more detail -

Payments-

columns could be

1- Insurance - HSA Fund - 3 Client

Adjustments-

columns could be

1- provider discount 2- deductible 3 co-pay/co-ins amount

Trying to explain to clients how the balance is calculated is really difficult without the data on the statement in front of each of you
