There should be a communication tab under the client, that can document conversations or comments about the account. Most offices have more than one person dealing with therabill, and communication is very important. Also, when a clinic has more than 10 patients it is hard to keep things straight as to what you said to the patient, or the insurance, or faxed something. A communication log would help keep things in chronological order, and who did what! Right now we use sticky notes, but that doesn't allow for other people to see my notes if the file is in my office!
