WebPT Ideas and Discovery

I have found that I have gotten several medicare denials due to reason code 16. When I call them they say that the ERA should have a remark code as well (an example is ma112). Medicare has refused to provide me with these codes over the phone and they state that it should be on the ERA. Without this information I am left with having to guess what is missing since the CMS-1500 form looks accurate.
