Add a time stamp to claims

By adding a time stamp, not just the date, we can see if the claims entered the system (by interfact or by hand). This is helpful to see if claims came in after hours and how long it is taking our billers to push the claims out.

    The Activity Log can be used to see the date, time, and user that added sessions into the application.
