Allow a place to enter notes on an ERA, at the ERA level and the patient level

To display a note about the ERA that maybe useful for others viewing it after the fact. There is plenty of real estate on the screen to do this. This would save time by not having to review what and where the issues is on the ERA.
Anyone needing this info on ERAs with issues or changes to an ERA.
It would allow the user add a note on the ERA or a note on the ERA at the patient level. This will not take away from anyone who doesn't use this feature but will help everyone who does use the feature.
