Add Customizable settings for DOS range | WebPT Ideas and Discovery

Add Customizable settings for DOS range and Resubmissions drop down under Bill Insurance.

The Bill Insurance screen now defaults a 3 month DOS range and selects the drop down to Hid All Resubmissions. This creates two additional clicks before any troubleshooting on can be completed. it consistently slows down the process to access the billing information.
