Add a copay to each visit

Make a way to add a copay to each visit. That way we can bill the patient for a missed copay even before the insurance processes the claim. Sometimes an insurance can delay a claim for quite some time and patient's do not like receiving a bill lon...

WC accounts to only link to Employment accident date info in Advanced tab.

at this time if patient has Workers Comp and Regular Medical Insurance -the Advanced Data is being pulled into both payers. Not just the WComp case. Need this to be more specific to case than it to pull for both type of cases.
