TAKE OUT NICKNAME FROM DOCUMENTS Merged

I have been complaining about this since May. Now due to your updates, I get to see the nickname on the top of every page. PLEASE take out the field that pulls the nickname onto a medical document!!! Example: Albus Headmaster Dumbledore. Headmaster is the nickname not a legal name or Hippa verification.
