OT documentation reconciliation

Hands/UE-- Current profile has errors and exclusions (Wrist/Hand)- Only digit that has a CMC joint is the thumb - it currently is included for all in the joint mobility section- No place for hand ROM (AROM/PROM needed) measures for digits- wrist/forearm only included- I have been using joint mobility section as the specific joints are listed here to limit my free typing. This is still quite time consuming.- Each consecutive note (including daily notes!!) should include objective measure ROM sections - with more complicated cases including multiple digits/joints sometimes ROM measures take more than one visit.- Passive joint mobility DOES NOT need its own section truly. Can be included as comment box in ROM measure section- Special tests should include Tinel's, Phalen's, Snuff box & Scaphoid shift (Scaphoid Fracture Screen), and Finkelstein's


Neuro/Cognition--There is not currently a profile for this that is appropriate or encompasses needs correctly-No cognitive assessments included in WebPT currently- This type of profile could be used for stroke, ADHD, dementia/alzheimer's, TBI/concussion- Should include sections for memory, attention, executive functioning (adaptable/abstract thinking, planning, self-regulation, impulse control, working memory, time management, and organization), vision, coordination- For stroke patients should include full body ROM (FM and GM), strength, functional communication, ADLs- My suggestion is to make a Neuro Profile which includes all the above aspects as the diagnosis I am thinking of using them for often have a lot of overlap deficit wise because of their basic neurological basis
