Show client's DOB & age at top of in daily note/documentation.

Please show the client's DOB and current age in yr/mo at the top of any clinical documentation-the daily note, evaluation note, etc. This is helpful in a pediatric practice to remember the exact age of each client, especially when shared between providers/SLPs.
