Revise the Client Invoice Statement to match Explanation of Benefits forms

If the Client Invoice had the same column headings as Explanation of Benefits forms then it would be easier for providers and clients to track charges, what's allowed, what's written off, what the co-pays are and what specifically the client owes. As it is now, the column headings are ambiguous and misleading.
