The Benefit Verification Report does not provide the Company name (or TIN). So when the report is worked across a member with multiple companies(EMR databases) or TINs then the user can't easily understand which clinics belong to which companies or to aggregate the report by company.

Acceptance Criteria:

1. Add Company Name column

   1. This should pull from the Company Name listed in the EMR Admin Slice

2. Add TIN column

   1. This should pull from the TIN that is listed in the EMR Admin Slice
