Lost patient percentage

what I'm looking for is a percentage of lost possible appointments, so no shows, cancels, and avaiable appointments that were not filled. Like a percentage of holes in the schedule, how many visits a Therapist could have seen altogether verses what the Therapist actually saw in a day. This would be a percentage for the entire day and would take into account how many possible appointments each therapist can see in a day verses how many the therapist actually sees in a day.
