We are also experiencing this same issue. I just finalized a new PN today (3/24) and the pt.’s name is still too close to the top of the page and gets cut off when we fax the document to the PCP as the fax stamp covers up the name on every page. We’ve received multiple calls from doctor’s offices and they are frustrated with not knowing which pt. these documents correlate to.

I would like to add on to my comment. I have taken a picture of what the fax looks like, as well. The information in the header is too high on the page and is constantly getting cut off on both sides of a fax. This is a fax FROM WebPT to physical fax. We have drawn a line to where the header is getting cut off on the fax. It appears that a fix in the location of the header can correct this problem. Also, it would be beneficial to have the name of the patient near all of the signatures on the final page.

Now, the doctor name is missing from the Plan of Care signature page. juts a blank line. What is going on?

Today, upon viewing an initial eval, the patient name isn't present on the document unless you choose to print it. There should be a patient name on every single document page either viewing or printing.

Also, If they do sign it and send back only the signature page (the final page) there is no name on it and we are unable to tell who they signed it for. The patient name should also be near the physician signature location.

Yes, we are having the same problem. We have had two doctor’s offices call us in the past week. They told us they can’t sign the POC because there is no PT’s name on it.
