Update online statements

Currently the email billing statements sent to patients include typos, are excessively wordy (complex) and appear in a format that we routinely get calls asking if the email is legitimate or if it is someone impersonating us - trying to hack their account. Here are some examples of another program we use that has a more simplistic, user friendly view.
